2 Upload Documentation
Documentation is professional diagnostic information prepared by a licensed clinical
professional such as a psychologist, clinician, specialist, medical doctor or other
qualified diagnostician.
Documentation is required to be uploaded directly to your online application or by using the link provided in the confirmation submission email. You are welcome to email or fax it directly to our department also, at disabilityservices@cocc.edu or fax at 541-318-3737.

3 Prepare for an Appointment
Once our department has reviewed your application and documentation, you will be contacted
directly to the number provided on your application to schedule the intake appointment.
Within this appointment you will be given your accommodation eligibility, along with
training on how to use your new disability services account. For further information

to the right.
